The child has not been evaluated by a licensed occupational therapist in the past 6 months or the treating therapist is requesting a full evaluation for the child. This includes a 1-hour in-person assessment using standardized and/or non-standardized tools/assessments, a write-up of evaluation findings and recommendations, and established goals. Fees include processing charge of 2.9% + $.30 per payment.

The child has had a full comprehensive occupational therapy evaluation in the past 6 months. This service will include a review of previous therapy paperwork and other pertaining documents, review of Trailroot Kids Intake paperwork, and 45-minute phone consultation. Previous goals may be carried out or changed/modified based on clinician and parent discussion of needs. A full evaluation may be recommended if needed. Fees include a processing fee of 2.9% + $.30 per transaction.

Individual occupational therapy package includes 10 75-minute sessions in a nature-based/community setting, 3 30-minute consultation calls or in-person meetings, and goals/treatment plan tailored to the child’s needs. $1,600 total in 3 monthly payments of $533.33 completed within 3 months. Fees include processing charge of 2.9% + $.30 per payment.
